By submitting this form, hereby grant and authorize The Mariners’ Museum and Park the right to take, edit, alter, copy, exhibit, publish, distribute, and make use of any Materials such as photographs, video, or audio recording The Mariners’ Photographer or Videographer created of me and/or my minor childrens’ likeness. I agree for these Materials to be used in and/or for promotional materials, including, but not limited to, online media, newsletters, flyers, posters, brochures, advertisements, fundraising, letters, annual reports, press kits, social networking sites, and other print and digital communications, without payment or further consideration. All purposes will be lawful and non-defamatory. This authorization shall continue indefinitely unless I otherwise revoke said authorization in writing.

I understand and agree that these materials shall become the property of The Mariners’ Museum and Park.

I hereby hold harmless and release The Mariners’ Museum and Park, its employees, including the photographer, contractors, and any third parties involved in the creation or publication of these materials, from all liability or claims by me, my heirs, or any third party in connection with my participation.

I hereby certify that The Mariners’ Museum and Park owns copyright to these Materials. I give The Mariners’ Museum and Park permission to organize the materials according to accepted archival principles; to create metadata, finding aids, and full-text search interfaces required for the preservation and discovery of the materials; to make the materials accessible to researchers and staff; and to use the materials in exhibits and displays, both physical and online. I agree that this material may be made available online under a Creative Commons Attribution 4.0 license (https://creativecommons.org/licenses/by/4.0/), which will allow others to share and adapt the material, as long as I am given appropriate credit.
